If further treatment is needed by another physician, a referral will be made from within the approved
network. If the employee stays within the network for treatment, bills will continue to be paid under
the insurance program.
If for any reason an employee chooses to go elsewhere for unauthorized treatment, the
insurance coverage will cease and any bills will be a personal expense.
• If you are out of work or need follow-up care as a result of your work-related injury/illness, you will be
contacted regularly by your nurse case manager to ensure your medical needs are met. Each of you is
an important member of the Renbrook community and the objective of this plan is to ensure that you
receive appropriate medical care in a timely fashion.
• The Renbrook School Plan includes an appeal process which is available to you should there be any
disputes or questions related to the level and/or quality of care you receive. Please see the
administrator of this plan in the Business Office with any questions.
FLEXIBLE COMPENSATION PLAN
Under Section 125 of the Internal Revenue Code, employees may voluntarily choose to participate in the
dependent care spending account and/or the healthcare spending account. Both accounts will reimburse
employees with pre-tax dollars for a variety of qualifying expenses. For example:
• Office Visit and Prescription Drug Co-Pays
• Other health care services not covered by the group plan
• Child and/or Dependent care expenses
Maximum contributions are limited to $5,000 per calendar year for dependent care and $2,500 for healthcare
spending accounts. The plan year is July 1 to June 30 of each year.
Effective July 1, 2009 an employee who has chosen to participate in the school’s Health Savings
Account program is INELIGIBLE to participate in the medical portion of the Flex program.
The program is administered by Discovery Benefits, (discoverybenefits.com) who provide claim processing,
payment and overall administration of the plan.
All monies left in an individuals’ account at the end of the plan year will be forfeited to Renbrook. Eligible
services must be incurred by June 30 of each year, and all claims must be submitted to Discovery Benefits
within 90 days after the end of the plan year (June 30); September 29.
GROUP LIFE INSURANCE
The school provides group life insurance, in an amount equal to one-time the employee’s cash salary, to all
benefit-eligible employees. SunLife currently administers the plan. Specific details of the plan are available in
the Summary Plan Description distributed to each participating employee. Employees are eligible for this
benefit on the first day of the month of employment.
