Critical Illness Insurance
Critical Illness Insurance provides cash to help pay for both medical expenses not covered by your medical plan as well as
day-to-day expenses that may start to add up — like rent, mortgage, car payments, etc. —while you are ill. With Critical Illness
Insurance, if you are diagnosed with a covered illness, you get a lump-sum cash benefit, even if you receive other insurance
benefits.
If you are diagnosed with any of the following critical

This plan includes a wellness benefit (once per covered person per calendar year)
equal to $50 if you receive any of the following screenings.

• Cancer screenings Heart Attack (Myocardial Infarction) 100%
• Cardiovascular Function screenings Carcinoma in situ 25%
• Imaging Studies
• Annual Examinations by a Physician Advanced Alzheimer's Disease 100%
• Immunizations Major Organ Failure Requiring

Transplant
This plan has a pre-existing condition limitation which applies to late entrants and
increases. Benefits are not payable when a date of diagnosis for a covered loss Coronary Artery Bypass (Graft or 50%
occurs in the first 6 months following a member’s effective date and the covered surgery)
loss is caused by, contributed to by, or occurs as a result of any of the following:
• a Pre-existing Condition; or complications arising from treatment or surgery Alzheimer's Disease 100%
for, or medications taken for, a Pre-existing Condition. Coronary Artery Disease 50%
• Pre-existing Condition: if, within the 6 months just prior to their Coverage
Effective Date, an Insured has an Injury or Sickness for which medical advice Skin Cancer $500
was given or treatment was received or recommended to be received during
