LONG TERM DISABILITY
Eligibility All active full-time employees working 30 hours per week in the
United States with the employer.
Benefit Amount 60% of monthly earnings to a maximum benefit of $15,000 per
month

• 2 Year Regular Occupation
• Zero-day Residual
• Accelerated Elimination Period
• Work Incentive Benefit during the first 12 months of disability
payments
Elimination Period 90 days; 30 day accumulation feature

Portability If you retire, reduce your hours or leave your Employer, you can
take this coverage with you according to the terms of the
contract.
Life and AD&D Benefit Reduction 65% at age 70; and 50% at age 75
