x When the you click on “Opt-Out” you are taken to a
confirma on page where you confirm that you do not wish to enroll
at this me.
x When the you click “Learn” you are taken to materials which tell you
more about the coverage available to you. You will not see the
“Apply” bu on un l you have gone through all of the “Learn” pages.
x When the you click the “Apply” bu on you will be
directed to the E-consent page where you must confirm your willing-
ness to conduct business electronically. Once you select “Yes – I
Agree” you will be directed to the Learn pages. If you choose “No – I
Do Not Agree” you have the op on of having a paper applica on
mailed to your home.

Step 5: Enrollment

When you begin enrolling, you will see a screen which provides important informaƟon
such as:
x Who can be covered by this policy
x How to waive coverage
x How to add a dependent
x The effec ve date of coverage

By following the screen prompts, you will be asked to answer ques ons, select coverage
amounts, add dependent coverage (if you choose to do so), confirm your elec on and
electronically “sign” the applica on.

If you wish to enroll over the phone or speak with a counselor,
you can do so by calling (866) 314-0355 between November 16

and ĞĐĞŵďĞƌ 5 from 8 a.m. to 8 p.m. EST.
