well
Age Well gives older people the support they need to look after their own health and wellbeing.
What is the Age Well Team?
Age UK Northamptonshire forms an integral part of a larger initiative which operates countywide. Funded by the Integrated Care Board, we work in partnership with NHS/ NHFT, Northamptonshire’s Adult Social Care Services, Northamptonshire Carers, Alzheimer’s Society and other health,
social and voluntary sector organisations. The project supports GP surgeries in Northamptonshire to meet the needs
of older people in a brand new way. By bringing different services together it provides a holistic approach, to support people who are frail, vulnerable and at risk of hospitalisation or crisis.
Age Well supports a total of 78 GP practices and covers 14 primary care
networks in Northamptonshire.
Age UK Northamptonshire’s Age Well team is made up of 13 members of staff
who work across the county to support people and their carers. We work in collaboration with health, social, and voluntary sector care partners to give people a voice and more say about the care and support they receive.

     An example...
The GP asked our Ageing Well Coordinator to visit Mr H who was struggling with his mobility and was falling regularly at home. After an in- depth conversation and listening to his concerns we arranged and facilitated an online meeting with his GP and other relevant professionals.
Several medical tests were ordered, and a UTI was discovered. We arranged a full benefit check, organised a lifeline pendant, a stairlift and bed lever installation, and a further walking aid was purchased. A full benefit check identified he was entitled to Pension Credit and Council Tax Support, and we helped Mr H to make the claim.
Thanks to our support the patient says “I feel okay, I feel better on my legs recently than I was before you
got involved, and feel I have a little bit more energy. I don’t know what I would have done otherwise.”
