Our Hospital Discharge and Community Team
The HD&CT provides short-term support for older people living with long-term health conditions, or after a stay in hospital.
The team helps individuals to
access ongoing services and
aims to reduce the likelihood of hospital re-admissions and enables people where possible to regain their independence.
Barry and Megan contacted
Age UK Northamptonshire after reading about the HD&CT in an Engage magazine. They weren’t sure what help was available locally as they were new to the area.
Barry’s recent knee replacement surgery meant he had a lot of questions about walking aids, grab rails and improving access to his home and garden. Karen Timson a HD&CT Support Worker visited Barry and

Megan at home to assess his needs. Karen referred him to the Age Well Team and to Community Occupational Therapy who were able to provide low-level home aids that would help him to live independently.
Megan decided she’d love to become a volunteer for Age UK Northamptonshire and Karen suggested Telephone Befriending might be just what she was looking for.
