This is to remind you that all ACI jobsites are required to maintain Code of Safe Practices & New Employee Safety
Orientation form for everyone’s safety acknowledgement and attention.

Please submit ALL COMPLETED CSP & NESO by email or hand in to Ana Victor, Front Desk, by Monday 11/17/2014.
If there is any additional CSP & NESP, that needs to be submitted along with Weekly Safety Meeting & Jobsite Safety
Survey each week.

ACI will commence mandatory ACI Safety First Sticker (ASFS) requirement at JOBSITE (HARD HAT) from November 5,
2012.
Any one who is working in your jobsite should have the mandatory “ASFS” on their Hard Hat before start working at
site.
Workers can exchange the signed Code of Safe Practices & New Employee Safety Orientation form (CSP & NESO) with
the “ASFS”
Please ensure keep the record of names who received the “ASFS”
ACI Field management can send the workers to their home anytime if someone doesn’t comply for this safety
requirement.
ACI Safety Admin will start jobsite inspection for this safety compliance starting from Tuesday, November 6, 2012.

ALL ACI Jobsites PM/PE/SUPER are required to PICK UP the “ASFS” (initially 100/PM) on Friday 11/2/2012 after 3PM at
ACI office
for distributing of respective jobsites from Monday, November 5, 2012.
