ACI encourages you to participate in Angeles Contractor, Inc. 401(k) Profit Sharing Plan effective January 1, 2017.
Please be advised the detail guideline & instruction as noted below:

A) You may choose ANY % of YOUR PAYROLL to contribute to your retirement account up to $18,000, or $24,000 if 50
years old or older.
B) ACI will MATCH 100% of YOUR CONTRIBUTION up to 3% of your payroll.
C) Please note that new plan starts with NO VESTING for PAST SERVICE.
D) ACI’s matching contribution will be credited at VESTING of 20% for each completed year STARTING 2017.
E) For enrollment, you may choose to fill out the enrollment form in page from page 35 to 43.
F) You may also choose to enroll via ONLINE or over the PHONE and the Instruction is on page 45 however, ACI
recommends enroll via hardcopy submission.
G) If you choose NOT to enroll, simply complete page 35 & 36 by marking “I ELECT NOT TO ENROLL” and SIGN.
H) You have until December 23, 2016 to enroll otherwise your enrollment may delay to 2/1/2017.
I) Also any change you make in the future, it will take an effect on FOLLOWING MONTH, such as payroll deduction %
change, beneficiary change, and etc….
