It has been 2 years since we have introduced 401(k) retirement plan in January 2017.
There has been no change in plan design, but we would like to remind you that there’s certain fee associated with your 401(k)
plan and Qualified Default Investment Alternative (QDIA).
Same Participant fee Disclosure and QDIA notice have been provided along with enrollment booklet previously.

Attached to this email are important notice and disclosure. Please read each document carefully.

 Participant Fee Disclosure – The Participant Fee Disclosure provides information related to the plan’s investment
options including fee and expense information and information to help you make your investment decisions by noting
any transfer restrictions that may apply to an investment option in your plan.

 QDIA Notice – The Qualified Default Investment Alternative for our plan is Fixed Interest Account.

You may register for your account via the web at www.oneamerica.com or via TeleServe ® (Participant Service Center) at 1-
800-249-6269.
