Revelations Health Care Group is committed to a comprehensive benefit program that helps our
employees stay healthy and feel secure while maintaining a quality work/life balance.

Revelations Health Care Group is pleased to offer our Group 2 full-time employees two medical
plans with Pan American that includes Healthcare2U (DCP) Direct Primary Care benefit that offers
Unlimited $10 PCP Doctor Visits and Unlimited $25 Urgent Care visits, see pages 4 - 11 for more
information. Our medical offers Unlimited Preventative Care coverage with NO Copay,
Deductibles, or Out of Pocket Costs. Our benefit package also includes a new Voluntary Dental,
and Vision with Mutual of Omaha. NEW this year is Voluntary Life with matching Accidental Death
& Dismemberment. Employees can apply for $150,000 with NO Health Questions, Spouses $30,000
and Children $10,000. Also new this year is Critical Illness with NO Health Questions meaning you
cannot be turned down. Finally, we are offering with Mutual of Omaha On & Off the Job Accident
Coverage for employees and dependents.

Revelations Health Care Group contributes a portion of the monthly employee only premium.

necessary contact information. This booklet will not serve as an ID card or verification of benefits at
a provider’s office, but can provide assistance if needed. Refer to page 21 for instructions on how
