         Adolph’s  Litho  Services  is  committed  to  a  comprehensive  benefit  program  that  helps  our
         employees stay healthy and feel secure while maintaining a work/life balance.

         Adolph’s  Litho  Services  is  pleased  to  offer  you  a  medical  plan  to  meet  your      individual
         needs.  We also offer our employees voluntary dental and 100%   Employer Paid,  $25,000
         Basic Life and AD&D coverage. Telehealth continues  to provide 24/7 doctor access for a
         low copay.

         Adolph’s Litho Services pays 100% of the cost of the employee’s medical plan and ALL, but
         $25 per pay period for dependent cost.  Our voluntary dental is an affordable option for the
         employee and employee’s dependents.

         Adolph’s  Litho  Services  provides  you  with  an  401(k)  plan  the  opportunity  to  save  for
         retirement  on  a  tax  -  advantaged  basis  by  “electing  to  defer  a  percentage  of  your
         income”.    We match your contribution $1.00 for dollar up to 3%.    We will match your
         contributions of more then 3% percent and up to 5% percent of your pay with $0.50 for
         each dollar contribute for the next 2%. These funds are 100% vested.

         Please  retain  a  copy  of  this  guide  in  order  to  review  coverage benefits,  cost  information,
         and retain necessary contact information on  page 13. This booklet will not serve as an ID
         card or verification of benefits at a providers office, but can provide assistance if needed.
