HOW TO ENROLL
The  rst step is to review your current bene t elections in the Bene tSolver system. Verify your personal information and make any necessary changes. Select your new 2017 bene t elections. Once Open Enrollment ends, you will not be able to change your elections until the next open enrollment period unless you experience a quali ed change in status.
ELIGIBILITY
Employees who work a minimum of 30 hours a week are eligible to enroll in all of our bene t plans. Changes made as part of Open Enrollment will be effective as of August 1, 2017. Our comprehensive program provides you with the opportunity to select the best  t for you and your family.

Requirements
Your Spouse
Must be your legal spouse for federal tax purposes including state recognized common-law spouses and same gender spouses.
Your Domestic Partner
Same and opposite gender*
Your Children, Including: Biological and adopted children
Disabled children who depend on you for support
Children you are required to cover under the terms of a Quali ed Medical Child Support Order (QMCSO)
Must be:
Under age 26
A disabled child of any age
* In order to include a Domestic Partner on our bene ts Plans, you must submit an Af davit of Domestic Partnership and a Declaration of Tax Status. If your domestic partner does not meet the IRS de nition of an eligible dependent, additional contributions for his/her coverage will be taken on an after-tax basis.
