PURPOSE
To help teams think about the institutional trappings in a traditional nursing home and how they could be replaced so it becomes home.
DESCRIPTION Participants make a list of institutional trappings in their nursing home and then brainstorm about how to replace them with the makings of home.
PARTICIPANTS Teams of 4-5

Steps:
• Set the team up by announcing that they are here to do an “Extreme Makeover” of the traditional medical model nursing home. Just like in the television series, the teams need to make the case for why the team should come to your nursing home and spend a week doing a makeover. Each team should take 7 minutes to capture on sticky notes all the medical model trappings still present in their nursing home. One item per sticky note.
• After the 7 minutes have passed, ask for a representative from each team to take their sticky notes to the front of the room, read each of them off and stick them to the wall or flip chart to start creating a list. Have all the team representatives do this, not posting repeats.
• Once all the items (sticky notes) are up on the wall or flip chart, the teams change their roles. They are now the Extreme Makeover team and are tasked with coming up with ideas on how to makeover the institutional trappings so the nursing home is transformed into home.
• Divide the total number of sticky notes by the number of teams. One member of each team is sent up to choose that number of sticky notes. They need not be the sticky notes that team presented.
• Give the teams 20-25 minutes to come up with ideas for each sticky note. Someone in each group can take notes on the ideas. At the end of the time, have the teams share their ideas with the whole group. If the leader of the exercise has some other ideas to add to the suggestions, you can do so, but be sure not to dominate the feedback.
