When you perform a root cause analysis

It is important to know the views of both process customers and suppliers.
If possible, hold a meeting with everyone involved to identify together the possible causes of the problem from all angles.
Keep in mind all the time that the focus is on the process, not on people. Important Note: We seek to improve the process, not change behavior or find guilty people, this helps things flow better, find the root cause and seek a permanent solution.
