At-Home Compression Device Order Process
For DVT Prevention
Simply fax or email the following items at least 5 days prior to surgery:
☐ Written Order / Rx ☐ Demographics Sheet ☐ Last Clinical Note
- ICD-10 Code - Insurance Info. Note must include:
- Surgery Date - Patient Mailing Address “I am prescribing an SCD unit
- Patient Phone Number for homecare use to prevent
Option: Submit a one-time standing DVT. Patient will not be fully
order, and include ICD-10 code and ambulatory after surgery and
surgery date on clinical note. is high risk for DVT.”
