D. DENTAL INSURANCE
Dental insurance is offered to full-time, regular employees as defined by Molzen
Corbin in Section III. Plan details may be obtained from the Human Resources
Department.
E. LIFE INSURANCE
Life insurance is provided to full-time, regular employees as defined by Molzen
Corbin in Section III. Plan details may be obtained from the Human Resources
Department. Voluntary Supplementary Life Insurance Coverage for employee and
dependent(s) is optional and paid by employee.
F. LONG-TERM DISABILITY PLAN
Molzen Corbin provides a long-term disability plan for temporarily disabled
employees. Plan details may be obtained from the Human Resources Department.
G. PROFIT SHARING PLAN
Molzen Corbin has established a 401(k) profit sharing retirement plan which it may
contribute to on an annual basis. Employees are eligible for the Plan after at least
1,000 hours of service, so long as they have reached age 21. Employees may enroll
on January 1st or July 1st after they are eligible. Details concerning contributions to
and participation in the plan are included in the Summary Plan Description which is
provided to all new employees when they become eligible to participate and available
from the Human Resources Department. The funding of the plan is determined
annually by management at its own discretion and in accord with applicable law.
