                        JOHN ITAMURA, M.D.                ORTHOPEDIC SURGEON

                         John  Itamura,  M.D.,  is  a  Board  Certified  Orthopaedic  Surgeon  at  the
                       Kerlan Jobe Institute/ Cedars Sinai Medical Center at The White Memorial
                       Medical Center.  He specializes in open and arthroscopic reconstruction
                       of the shoulder for shoulder instability and the rotator cuff, disorders of
                       the sternoclavicular and acromioclavicular joints, pathology of the biceps,
                       primary  and  revision  arthroplasty  of  the  shoulder  and  elbow  and  the
                       treatment of complex periarticular fractures of the shoulder and elbow.
                         Dr. Itamura is a native of Los Angeles and earned his Bachelor of Science
        from  the  University  of  Southern  California  in  1985.    He  graduated  from  the  University  of
        Southern California School of Medicine in 1989.  He completed his Residency in Orthopaedic

        Surgery in 1989 at the University of Southern California.  In 1995 he completed a Fellowship in
        Shoulder and Elbow Surgery at the WB Carrell Clinic in Dallas, Texas.  In 1995 he was full-time
        faculty on the Upper Extremity Trauma and Sports Services at Keck School of Medicine at the
        University of Southern California.  In the fall of 1997, he took a sabbatical in Upper Extremity
        Reconstruction at the Massachusetts General Hospital.  He was an active member of the Sports
        and Hand and Upper Extremity Fellowship Program.  In 2011, Dr. Itamura joined the Kerlan Jobe
        Institute.  He is the Director of the Shoulder and Elbow Fellowship, and an active member of the
        Sports Fellowship faculty.  He is an active researcher and has published numerous peer and non-
        peer reviewed papers and book chapters.  He regularly lectures locally, regionally, nationally
        and internationally, and serves as a peer reviewer for numerous Orthopedic journals.
          He is a member of the American Academy of Orthopaedic Surgeons (AAOS), The Wilson Bost
        Society, and the American Shoulder and Elbow Surgeons (ASES)
          This is his first season with the Los Angeles Angels.  He has served a number of professional,
        collegiate  and  high  school  athletic  teams,  including  the  Los  Angeles  Dodgers,  University  of
        Southern California and California State University, Los Angeles.



                        CRAIG MILHOUSE, M.D.                  TEAM PHYSICIAN

                         2019 represents the 26th season as the Angels Team Physician for Dr.
                       Craig Milhouse.  Dr. Milhouse’s current professional duties also include
                       his private practice in internal and athletic medicine in Orange, CA and
                       as a medical consultant/physician for the Fullerton Junior College Athletic
                       Department.    He  previously  worked  for  10  years  at  the  Kerlan-Jobe
                       Orthopedic Clinic in Anaheim, CA .
                         A native of Detroit, MI, Dr. Milhouse received his bachelor of science
                       degree  from  the  University  of  Michigan  in  1976  and  medical  degree
        from Wayne State University School of Medicine in 1980. He completed his medicine/surgery
        internship  at  St.  John’s  Hospital  in  Detroit  (1980-81)  and  his  internal  medicine  residency
        at Huntington Memorial Hospital in Pasadena, CA (1981-84). Dr. Milhouse served his sports
        medicine  fellowship  at  the  Kerlan-Jobe  Orthopedic  Clinic  in  Inglewood,  CA  (1990-91)  and
        received his board certification in internal medicine in 1984.
          Dr.  Milhouse  has  worked  as  a  team  physician/medical  consultant  for  several  local  sports
        organizations, including the Anaheim Ducks & Honda Center (1993-2015), Los Angeles Rams (1993-
        95), University of Southern California (1988-93), Occidental College (1992-93), Anaheim Bullfrogs,
        Anaheim Piranhas and Anaheim Splash.  In 2003, Dr. Milhouse served as the Medical Director for
        the World Gymnastics Championships and was a voluntary member of the Tournament of Roses
        for over 25 years. He and wife, Pamela, have two sons, Jonathan and Alex. In 2016, they became
        grandparents with the birth of grandson, Jaxon Ryan.
