Planning Process
The planning process consisted of two phases funded with Center for Disease Control (CDC)
funds administered by ISDH. Pre-planning was done to design a planning process to gather the
perspectives of diverse stakeholders. Pre-planning began in the fall of 2016 and included the
following activities: H

• Review of the current statewide asthma plan
• Review of state and national priorities:
• Guidance and priorities of ISDH

• The CDC Comprehensive Asthma Control Funding Opportunity Announcement (FOA) and
logic model O
• Healthy People 2020 Goals, Objectives, and Measures related to asthma

• Discussions with the InJAC Coalition Coordinator
• Input from InJAC members at the InJAC Quarterly Meeting in October 2016
• Data review, including:
• ISDH asthma-related data
• Crosswalk of data priorities and available data Albuterol Sulfate
