                       APPEAL TO HEARINGS COMMITTEE
                       If you (or your spouse) are dissatisfied with the decision of
                       the Appeals Committee, you may request a hearing before the
                       Hearings Committee, designated by the Trustees. This request
                       must be made in writing and must be received by the Trust within
                       30 days following the date you receive notice of the Appeals
                       Committee’s decision.

                       The Hearings Committee will conduct a hearing within 30 days
                       of the receipt of the appeal unless more time is needed, or unless
                       you (or your spouse) agree to a different schedule. If more time
                       is needed, you (or your spouse) will be notified. At the hearing,
                       you (or your spouse) will be entitled to present your position
                       and any evidence. You (or your spouse) may be represented
                       before the Hearings Committee by an attorney or by any other
                       representative of your choosing.

                       You (or your spouse) will be notified of the Hearings Committee’s
                       decision within 5 days of the date a decision is made. The
                       Hearing Committee’s decision is the final and binding decision of
                       the Board of Trustees and no further appeal is available under the
                       Plan. If the Hearings Committee upholds the denial, the Trust will
                       send you a written notice which contains the following:

                       Â   The specific reasons for the Trustees’ decision;

                       Â   References to the Plan provisions on which the denial is based;

                          necessary for you to perfect your application and an
                          explanation why such material or information is necessary;

                       Â A statement that you are entitled to receive, upon request
                          and free of charge, reasonable access to, and copies of, all
                          documents, records and other information relevant to your
                          application;
