     If you’re a full-time employee at City of North Las Vegas,
     you’re eligible to enroll in the benefits outlined in this guide.
     The following family members are eligible for coverage:
       •  Legally married spouses;
       •  Same sex and opposite sex domestic partners with valid
          registration from the Nevada Secretary of State
       •  children, step-children, children placed for adoption, and
          children under your legal guardianship per a court order
          as long as the child is under the age of 26
     Dependent Documentation Requirements:
     The following supporting documentation must be provided
     when adding dependents to the plan.
       •  Spouse: Marriage Certificate & Social Security Card
       •  Domestic Partner: Valid registration from the Nevada
          Secretary of State and Social Security Card
       •  Children/stepchildren: Birth Certificate or court-
          appointed guardianship certificate and Social Security
          Card

     New hire employees are eligible for benefits on the 1  of the
     month following their full time date of hire.
     The Annual Open Enrollment Period runs from (dates vary).
     The benefits you elect during Open Enrollment will be made
     effective January 1, 2020 through December 31, 2020.

     You must enroll using City of North Las Vegas online
     enrollment system called Benefitfocus.  See Page 16 of this
     benefit guide for complete instructions.

     The first step is to review your current benefit elections.  Verify
     your personal information and make any changes if necessary.
     Once you have made your elections, you will not be able to
     change them until the next open enrollment period unless you
     have a qualified change in status.

     Unless you experience a life-changing qualifying event, you
     cannot make changes to your benefits until the next open
     enrollment period.  Changes to your coverage when a
     qualifying event occurs must be made within 30 days of the
     event.  Qualifying events include things like:
       •  Marriage, divorce or legal separation
       •  Birth or adoption of a child
       •  Change in child’s dependent status
       •  Death of a spouse, child or other qualified dependent
       •  Change in residence
       •  Change in employment status or a change in coverage
          under another employer-sponsored plan
