ILLINOIS CRISIS PREVENTION NETWORK
The Illinois Crisis Prevention Network (ICPN) is a collaborative partnership between Trinity
Services and Hope. ICPN is grant funded by The Illinois Department of Human Services
Division of Developmental Disabilities and consists of six (6) Support services Teams
located throughout the state. The teams are designed to support intellectually disabled
individuals who are experiencing complex behavioral and/or medical challenges by providing
community-based support and consultation services to the individuals and their COLLABORATIVE
families/care providers with the goal of maintaining and promoting successful
community living.
