             Basic Life/AD&D Benefits
             Provided by UNUM




                  Basic Life/Accidental Death & Dismemberment
                    This benefit is 100% paid by the employer.
                    Benefit amount is 2 x annual salary to a maximum of $300,000 for eligible
                     employees.
                    Guarantee issue amount is $300,000 (no medical questions).
                    This benefit amount will reduce by 50% at age 70, and
                          terminates upon retirement.
                    Dependent coverage is available for spouse in the amount of $5,000; child
                     age 14 days to 6  months $2,000; Child age 6 months up to age 19, or 26 if a
                     fulltime student $2500.




                  Voluntary Life Benefits

                  Provided by UNUM

                 Voluntary Life/Accidental Death & Dismemberment
                   This benefit is 100% employee paid through payroll deductions.
                   Employee coverage is available in $10,000 increments to a
                       maximum of $500,000.  See policy certificate for reduction
                       schedule.
                   Spouse coverage is available in $5,000 increments, equal to 100% of the
                     employees election.
                   Dependent child(ren) may have coverage equal to $10,000 without proof
                     of good health.
                   Guarantee Issue amount  for the employee is  $150,000; spouse $30,000;
                     child(ren) $10,000.
                   Spouse/dependent child coverage is only available if the
                       employee has purchased voluntary coverage.
                   Rates for employee and spouse are based on the age of the
                       Employee.  Dependent child(ren) rate is $0.70 for $10,000 of
                       coverage. Please see your HR Administrator for details.
                   Future applicants are subject to medical underwriting.
                   This benefit is portable and convertible.
