Plan 2:
 Long Term Care Facility-100% of Facility Monthly Benefit Amount
 Total Choice Home Care-50% of Facility monthly Benefit Amount
Plan 3:
 Long Term Care Facility-100% of Facility Monthly Benefit Amount
 Professional Home and community Care-100% of Facility Monthly
Benefit Amount
 5% Simple Inflation Protection
Plan 4:
 Long Term Care Facility-100% of Facility Monthly Benefit Amount
 Total Choice Home Care-50% of Facility monthly Benefit Amount
 5% Simple Inflation Protection

Facility Monthly Benefit Amount is a minimum or $2,000 per month up to an overall
maximum of $6,000 per month in $1,000 increments.

Facility Benefit Duration is available in 3 years, 6 years, or Lifetime. Lifetime benefit
durations require medical underwriting.
