            Provided by Unum, this Benefit is paid 100% by Members Credit Union.
            Coverage equal to 2.5 times annual salary to a maximum of $500,000.  Term life reduces as you age
            with the first reduction at age 65 to 65% and reduces to 50% at age 70.   This benefit includes
            Waiver of Premium if you are disabled during the life of this policy.  Conversion and Portability are
            included.

            Provided by Unum, this benefit is paid 100% by the employee through payroll deductions.

            Employee coverage is available in increments of $10,000 up to a maximum of $500,000 OR 5X your
            salary, whichever is less.  Guaranteed issue amount lesser of current amount or $70,000 at
            enrollment.  See policy certificate for reduction schedule.  Spouse coverage is available in
            increments of $5,000 to a maximum of $500,000, but not to exceed 100% of the Employee’s
            coverage election.  Guaranteed Issue amount is $25,000.  Dependent child(ren) coverage is
            available in increments of $2,000 to a maximum of $10,000 (age  6 months to age 19, or 26 if full-
            time student).  For live birth to 6 months, coverage equal to $1,000. Child coverage amount cannot
            exceed 100% of Employee’s coverage election.  Spouse/dependent child coverage is only available

            if the employee has purchased supplemental coverage.  Employee rates are based on employees’
            age.  Spouse rates are based on spouse age.
