All of the information contained in this booklet is provided to outline some of the major features of
the Member’s Credit Union Benefit Plans. It is intended to be a brief overview only. Full Summary
Plan Descriptions (SPDs are available by contacting Human Resources). In the event that the
information in this booklet varies from the information in the SPDs, the provisions outlined in the
SPD will govern.
Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plans
offered at Members Credit Union you will receive an
Medical/Prescription Drug ID card from United Health Care (UHC). You may
request additional cards by contacting UHC
customer service at 1-866-633-2446.
If you enroll in the Dental plans offered at Members
Credit Union, you will receive an ID card from Delta
Voluntary Dental Dental. You may request additional cards by
contacting Delta Dental’s customer service at 1-800-
662-8856.
If you enroll in the voluntary vision plan at Members
Credit Union, you will receive an ID card from Blue
Voluntary Vision 20/20. You may request additional cards by
contacting Blue 20/20’s customer service at 1-855-
400-3641.
