Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plan offered
Medical/Prescription Drug at Allegacy, you will receive an ID card from Blue Cross NC.
You may request additional cards by contacting Blue Cross
NC customer service at 1-877-258-3334.
If you enroll in the Voluntary Dental plan offered at
Allegacy, you will receive an ID card from UNUM. You may
request additional cards by contacting UNUM customer
Voluntary Dental
service at 1-888-400-9304. Please note that the ID card
flyer you receive provides a list of a few in-network
dentists for your information; however, you are not
required to use an in-network dentist for services.
If you enroll in the Voluntary Vision plan at Allegacy, you
Voluntary Vision will receive an ID card from Superior Vision. You may
request additional cards by contacting Superior Vision
customer service at 1-800-507-3800.
