HRA Medical Plan





               You are eligible for the employer provided HRA funds if you are enrolled in the HRA medical plan. The
               health reimbursement account is not available with any other medical plans.

               The Health Reimbursement Arrangement is an additional benefit that is fully funded by HPCC that
               assists with the deductible and out-of-pocket expenses for those enrolled in the HRA medical plan.

               How does the HRA work?

                     $2,500 employee deductible
                     $5,000 family deductible
                     No copays for office visits or prescription drugs
                     Once deductible is met, plan pays 100% of medical expenses
                     Hospice & Palliative CareCenter contributes:
                          o  $500 for Employee Only
                          o  $500 for Employee/Spouse
                          o  $700 for Employee/Children
                          o  $700 for Family

               Enrolled participants will receive a HRA debit card. This debit card may be used to pay for
               medical and prescription drug expenses with your HRA monies.

               Rollover = Current year allocation +  one year.
