Basic Term Life/AD&D

This benefit is 100% paid by Hospice & Palliative CareCenter for all full-time
employees working 30+ hours per week.
Coverage equal to $20,000 Includes conversion privilege
Term Life reduces as you age with Includes portability

Voluntary Term Life/AD&D
Group# 405207
This benefit is 100% paid by employee through post tax payroll deductions.
Employee: You may purchase coverage in units of $10,000 to a maximum of $200,000
without medical evidence of insurability (If you enroll when you are first eligible. If you do
not enroll when you are first eligible, medical evidence of insurability will be required).
Coverage over $200,000 (up to a maximum of $500,000) is available with medical evidence
of insurability. Your Election is limited to 5 times the Employee’s annual salary.
Spouse: If you elect coverage on yourself, you may purchase coverage for your eligible
spouse in units of $5,000 to a maximum of $25,000 without evidence of medical
insurability. (If you do not enroll when you are first eligible, medical evidence of insurability
will be required). Coverage over $25,000 (up to a maximum of $250,000, but no more than
100% of the employee elected amount) is available with medical evidence of insurability.
Children: If you elect coverage on yourself, you may purchase coverage for your eligible
children between the ages Live Birth and 19 years old (to age 26 if full-time student), in the
amount of $10,000. Please note that the benefit for children from live birth to 6 months is
$1,000. Dependent children coverage may not exceed 100% of your coverage amount.
Rates are age-banded. The rate for spouse coverage is based on your age. Please see rate table.
Your cost for coverage may increase on your June 2018 paycheck due to aging into the next 5
year age band.
