Additional Benefits




               Retirement Plan 403 (b)

               The Hospice & Palliative CareCenter 403(b) Retirement Plan has been adopted to provide you

               with the opportunity to save for retirement on a tax-advantaged basis and to provide additional
               income for retirement. You are eligible to enroll and contribute to your 403 (b) plan upon date
               of hire. The plan provides for discretionary matching contributions on salary deferral
               contributions in an amount to be determined by Hospice & Palliative CareCenter. You are fully
               vested after three years of employment. The Summary Plan Description (SPD) contains valuable
               information regarding the specific details of the plan. The SPD is available to all employees via
               the employee intranet (wsw.hospichub.info) or by contacting Human Resources.


               Educational Assistance Program

               After one year of employment, Hospice & Palliative CareCenter will reimburse eligible
               employees for qualified educational expenses up to $2,500 per calendar year. Please contact
               Human Resources for further information.

               Employee Assistance Program (EAP)


               McLaughlin Young Employee Services is available to assist you and your family members with a
               variety of personal problems that may affect job performance. The program addresses such
               problems as, but not limited to: depression, anxiety, stress, other psychological issues, struggles
               with drug and alcohol abuse, marital and family concerns, aging parents, relationship
               difficulties, and financial strains. The EAP program is available to you 24 hours a day, seven days
               a week by calling 1-800-633-3353. The program is strictly confidential and all costs are paid by
               HPCC so there is no charge to you or your family member(s) using EAP services. Human
               Resources can provide you with additional information regarding this service.


               Paid Time Off (PTO)

               PTO is available to employees working a regular schedule of 20 or more hours per week. PTO is
               accrued based on date of hire, hours worked and longevity. Please refer to the Employee

               Handbook for the actual accrual schedule.
