Long-Term Disability with Lincoln Financial Group

 This benefit is paid 100% by DataMax Corporation for eligible employees
 Benefit amount is 60% of insured’s monthly earnings
 Maximum monthly benefit: $5,000

 Minimum monthly benefit: $50
 Elimination period: 90 days of disability due to the same or related Sickness
or Injury (no benefits during this time), which must be accumulated within
a 180 day period
 Cost of living percentage: 3% maximum.

 Maximum benefit period is to Social Security Normal Retirement Age, or
the maximum benefit period shown below (whichever is later)
