Identification Cards
You will receive identification cards when you enroll for the benefits listed below.
If you enroll in the Medical/Prescription Drug plan
offered at DataMax Corporation you will receive an ID
card from United Healthcare (UHC). You may request
Medical/Prescription Drug
additional cards by contacting UHC customer service
at 1-866-633-2446 or by visiting the UHC website at
MYUHC.com.
If you enroll in the Voluntary Dental plan offered at
DataMax Corporation, you will receive an ID card
Voluntary Dental from Lincoln Financial Group (LFG). You may request
additional cards by contacting LFG customer service at
1-877-275-5462.
If you enroll in the Voluntary Vision plan at DataMax
Corporation, an ID card from Superior Vision will be
Voluntary Vision mailed to your home address. You may also go online
to www.superiorvision.com to download and print
your ID card.
