Wellness Discount
In order to qualify for the Wellness Discount, you need to do two things:
Provide Human Resources with Proof that you have completed your Annual
Physical.
o Physical must be completed within the past 6 months (7/1/17 –
12/31/17) or be scheduled within the next 6 months (1/1/18 –
6/30/18).
o This Proof can come in the form of a carrier Explanation of Benefits
(EOB), Provider Bill or Billing Statement of signed letter from Provider.
Complete the Health Risk Assessment on United Healthcare’s website
“myuhc.com”
o Follow Directions as outlined on the Paylocity Benefits Enrollment Web
Portal
o Assessment must be completed by March 31, 2018
Tobacco-Free Premium Credit
Jones & Frank has a tobacco-free initiative meaning that a premium credit will be
applied to employees and covered family members enrolled in a Jones & Frank
medical plan who have never used tobacco products or who have refrained from
using tobacco products in the 90 days prior to the signature date on your 2018
Medical Enrollment in our Paylocity benefits enrollment Web Portal. The use of
tobacco includes all forms, whether smoking, chewing or any other methods of use.
Please complete the “Non-Tobacco Use Affidavit for Plan Year 2018-2019”
form on your 2018 Paylocity benefits enrollment Web Portal.
