How do I enroll?

You will utilize Paylocity’s Web Benefits portal to enroll. Instructions are as follows:

1. Click on the dark gray Web Pay on the top left-hand side of the screen.
2. Choose Enterprise Web Benefits from the menu
3. You should see your benefits home screen. Click on the “Enroll Now” tab
a. Once you are in the benefits site, you will need to confirm personal and family
information before moving on to your benefit elections.
4. Click on “View Plan Options”
a. You will be asked who you wish to be covered under the plan. Make the
appropriate choice and continue.
b. Choose the option you want to enroll in, then move on to the next benefit.
c. Once you have completed all benefits, click “Continue” on the right side of the
screen.
d. (You have the option to save and finish later.)
e. Once you “Continue”, depending on your elections, you may have some additional
questions to answer, such as beneficiaries, etc. After you complete and
“Continue”, you will be asked review all of your selections.
5. If all looks good, “Agree” and complete your enrollment.

Open Enrollment will be held from December 6, 2017 through December 12, 2017. The benefits
you elect during Open Enrollment will be effective from January 1, 2018 through December 31,
2018.

New hires must satisfy a waiting period of 30 days and then benefits will become effective the
first of the calendar month following 30 days of active full-time employment.
