Welcome!
Welcome to your 2018 employee benefits Open Enrollment!
We evaluate our plans each year to ensure they are cost-effective, market competitive and compliant with
the Affordable Care Act. Our program will incorporate some modifications for the 2018 plan year as
follows:
We will transition our medical plan to United Healthcare. You will have your choice of two plan
options. We will continue to offer an HRA to help offset your deductible.
Dental coverage will remain with Guardian. There will be two options. The “Low Plan” is the
same plan as 2017. Rates for the “Low Plan” are the same as in 2017. The “High” plan offers
Orthodontia coverage for children (under the age of 19) if you are interested in that coverage.
Vision benefits will remain with Avesis. These costs have decreased slightly while the benefit has
been enhanced. Frame frequency will be every 12 months instead of every 24 months. Also, the
contact lens allowance has increased from $130 to $150.
Life Coverage will now be with Guardian. Your cost for $50,000 basic term life/AD&D will remain
unchanged. The rates for the additional Voluntary Life Coverage are the same for 2018, unless
you age into a new 5 year Age-Bracket.
Disability coverage will now be with Guardian. Short-Term Disability rates remain the same.
Long-Term Disability rates are increasing slightly. You have the opportunity to elect coverage
without Evidence of Insurability (EOI) during this one-time open enrollment for 2018!!
Accident and Critical Illness coverage is also moving to Guardian. The Critical Illness plan with
Guardian covers Cancer in addition to other conditions. The Wellness benefit has increased from
$50 per year to $100 per year in the Accident plan. The Critical Illness continues to
reimbursement $100 for wellness benefits.
Universal Life coverage will no longer be offered on a Payroll Deduction basis. You will receive

notification from Allstate following January 1 to determine if you wish to continue this coverage
on an individual basis.
It is mandatory for you to access Paylocity Web Benefits and elect your Medical and Dental options.
If you are currently enrolled in either of these benefits and do not make an election you will be
auto-enrolled into the corresponding “Core Plan”. Your other benefits will automatically “roll over”
unless you decide to make a change.
