employer) will not be offered medical coverage through Jones & Frank. Please be sure to
complete and sign the “Spousal Exclusion of Medical Benefits Questionnaire” if you are electing
to have your spouse covered under our plan.

Eligible Dependents
Other members of your family may be eligible for coverage under Medical, Dental, Vision,
Who is Eligible? Voluntary Life/ AD&D, Accident and Critical Illness protection. Your eligible dependents include
your legal spouse and dependent child(ren) up to age 26, unless otherwise noted.
