GRANTEE MONITORING QUESTIONNAIRE:
ORGANIZATIONAL CAPACITY ASSESSMENT

Accepting an award from the International Potato Center (CIP) creates a legal duty for the
grantee to use the funds according to the award and applicable donor regulations.

The purpose of this questionnaire is to provide CIP with information needed to assess the
adequacy of the financial and accounting system and award administration capacity of a
prospective / current recipient of a CIP-provided award.

If you have an A-133 Audit for your most recent completed fiscal year, you do not need to
complete this questionnaire. Please forward a copy of your audit to our attention.

Each question should be answered as completely as possible, using extra pages if necessary.

Return your completed questionnaire to CIP as soon as possible. Delay in returning the
completed questionnaire may result in a delay in the issuance of the award.

Complete the questionnaire and checklist, sign and return it (along with any attachments) to
CIP. You may mail or e-mail the completed questionnaire to your CIP local contact (Country
Finance Specialist / Local Accountant or PMU Specialist).
