5D 1) Gross rent is $5,400/month and we are able to amortize 6 weeks free on a 13th month lease
starting 9/1/20 which means that our monthly payment will be $4,776.92 2) Guarantee that we will
have an option to renew our lease for another 13 months at the same gross rate of $5400 in
addition to an additional 6 weeks free on the second 13 months. 3) Please confirm that the below
sentence, in regards to free rent, applies to having the 1) roof that is fully furnished and finished and
usable 2) lobby that is fully furnished and finished with no scaffolding or plastic 3) gym is fully
furnished and finished and usable and 4) the laundry room and elevator are fully finished and
usable with no scaffolding or plastic, and lastly 5) the exterior of the building has no scaffolding or
construction work going on. This must all be the case or we will get free rent until the above is all
completed. "Will present the no rent clause until amenities are complete and furnished." 4) Given
the current pandemic, we would like occupancy of the apartment 1 week prior to our start date of
9/1/20. We would appreciate having occupancy on 8/25/20 as we will then have enough time to fully
move into the apartment during this difficult and unprecedented time.

recipients of any reports on myself, which On-Site has previously obtained about me within a two-year period preceding my request.
I consent to the delivery of all notices or disclosures required by law via any medium so chosen by Pixel or On-Site, including but not limited to email or other electronic transmission. I understand that all notices shall be
deemed received upon being sent.
By submitting this application, I certify that I have read and fully understand the disclosures and my rights detailed above and authorize On-Site to obtain, for the purpose of determining my eligibility for initial or continued
tenancy, and/or employment a consumer report and/or investigative consumer report and to disclose all information obtained by On-Site to Pixel, its affiliates, and assigns now and in the future. I also certify that I have

By submitting this application, I acknowledge that I would like to receive a copy of my consumer report and/or investigative consumer report.
