Voight Professional Services conducted an Annual Medical Gas Inspection at your facility on

November 17 -December 5 , 2017. We inspected your facility in accordance with NFPA 99.

We inspected all zone valve boxes, outlets, alarm panels, master alarms, Medical Air and Vacuum
Systems, and Medical Manifolds. We also did pipeline purity testing and particulate testing at each
zone in your facility.

All discrepancies have been noted and are provided in the following consolidated discrepancy list.
