Case Study
Mrs Green* (not her real name) had multiple falls over a few months and her family were having difficulty advocating for her support needs. The GP Social Work (GPSW) team helped the family to navigate My Aged Care
and liaised with the Home Care Package (HCP) service provider to review Mrs Green’s care. The team also requested additional supports for her.
The GPSW team completed an Advance Care Plan and safety plan to address Mrs Green’s falls risk. They requested for Mrs Green’s personal alarm to be updated and her emergency
plan reviewed. Additionally, the GPSW team assisted in arranging a functional Occupational Therapist assessment to get recommendations for assistive technology (suitable walker).
As a result, Mrs Green now has additional personal care, social support, and physiotherapy services through her HCP. Mrs Green’s HCP provider updated the
personal alarm equipment and the emergency plan.
After her OT assessment, she now uses an OT recommended walker, which has helped her with her posture and walking. Mrs Green reports that she feels
safe now in accessing the community.
