DISTRICT ACTION GROUP ON WHEELCHAIR DISTRIBUTION

WHEELCHAIR DISTRIBUTION SOP
1. Prospective recipient to be assessed at their respective location by our trained Assessor.
2. To submit completed Wheelchair Assessment Form with all details filled in and send via email to PP Raja Renno ( RC Melawati ) at rajarenno@hotmail.com and PP
Judy Chong ( RC Bandar Utama ) at judychong.rc@gmail.com
3. Upon receipt of completed forms, wheelchair will be assigned accordingly.
4. An administrative cost to defray GST charges of RM50 per wheelchair to be remitted by the requesting club to RC Bandar Utama as follow :

5. Respective clubs to bear the transport cost of wheelchair . It is advisable for Clubs to consolidate the trucking cost from Port Klang warehouse to the
respective drop points . The following Clubs have graciously agreed to be collection centres. They are :
a. PP Dr Bernard – RC Bdr Sg Petani
b. PP Aikky – RC Bukit Bendera
c. AG Dr Selva – RC Melawati
d. PP Wong Ming Hui – RC Indera Mahkota
e. PAG BH Lim – RC Kulim
f. PAG Rev Sam – RC Senawang
6. Completed assessment form must be submitted to PP Raja Renno and PP Judy Chong for administration record before release of wheelchair to Rotary club concerned
.
7. We will do a monthly wheelchair consolidation delivery to outstation by the 25th of the month for cost saving purposes .
8. If there is an urgent need, the respective club concerned to make other arrangement for collection with PP Raja ( 017 878 9635 ) or PP Judy ( 016 227 1308 ).
9. List of trained assessors attached for your referral.
