around 51-75% generally. noted on Namzaric Tray Card
and multivitamin supplement to help with disease Care Plan
process and to maintain intakes. will at this time
suggest to change house shakes to whole milk at
meals and consider flavoring with chocolate at
lunch and supper, and adding ice cream to lunch
and supper to maintain wts. monitor and refer to
RD as needed.

gain in last 2 months and is considered positive. PLEASE SEE
noted with dx of dementia, depressive disorder. SUMMARY FOR
also noted on diet plan of regular mechanical soft RECOMMENDAT
with melted margarine to hot vegetables at lunch ION
and supper, ice cream at lunch and supper, soup Tray Card
and sandwich at lunch and supper, soup served Care Plan
in a cup with handles, and house shakes at meals
for added calories. intakes vary and noted on
medication of cyprohptadine and remeron to
stimulate intakes. note as of 7/31 was started on
eldertonic to also help with intakes. at this time
no diet change, however if appetite increases
please d/c house shakes. refer to RD as needed.

diet plan. has dx of diabetes, however not on any PLEASES SEE
diabetic medications. also reported with overall SUMMARY FOR
good intakes. noted in addition to diabetes, RECOMMENDAT
resident with dx of paranoid schizophrenia and is ION
showing gradual wt gain. will at this time suggest Tray Card
to please change diet plan to regular with skim Care Plan
milk, fruit or 1/2 portion of regular dessert to
control wts and disease process. monitor and
refer to RD as needed.
