100#. noted with dx of schizophrenia, adult
failure to thrive, dementia, psychosis. remains on
diet plan of regular mechanical soft, with melted
margarine to hot vegetables at lunch and supper,
yogurt at meals, ice cream at lunch and supper,
clear liquid supplement at meals. noted also on
medications of Namenda, multivitamin with
mineral, megace and remeron to maintain
disease process and intakes. at this time wts
seem stable and no changes requested. monitor
and refer to RD as needed.
