AUTHORIZATION to Release Official School Records
In order to process any college or prep school applications, as well as the Bright Futures Scholarship applications, the following statement must be signed and on file in the guidance office. PLEASE RETURN THE SIGNED FORM TO THE GUIDANCE OFFICE AS SOON AS POSSIBLE.
Transcript and Standardized Test Scores Release
As part of the college or prep school application process and the Bright Futures Scholarship process, I authorize the release of a copy of my official transcript containing a list of courses, grades earned and a record of standardized testing as well as any other educational records to the extent required or requested by the educational institutions to which I apply.
I understand that official Gulliver School, records include only documents found in a student’s cumulative file and confidential file. Records and documents prepared by another institution, other than transcripts, that may be in Gulliver’s possession will not be released by Gulliver without specific written authorization from the author of those documents. Final transcripts will not be sent until all financial obligations are met.
I understand that any and all information and documents contained in the official files may be disclosed to a college, university or other school if a parent signs a release in favor of such college, university or other school, and if so, the release is effective on a “going forward” basis so the disclosures can be supplemented to provide notification of subsequent significant events.
I recognize, ultimately, I must have my SAT I and II scores and my ACT scores transmitted officially to the colleges by the testing agencies. I understand that I must have my AP scores sent directly to the college or university I will be attending. I also authorize the release of records to college coaches as well as to scholarship competitions when appropriate.
