                           Emergency Contact


                                   Information




        Please fill out the electronic form linked below to update your emergency contact information. We
        have provided some questions and answers as to why this is important for each employee to
        complete.

        What are we collecting? How are we using it?
        The form is separated into three sections.

            1.  The Employee Information section is designed to collect information about who you are and
               where you work. This information is used to update our 9-1-1 database so that when you call
               9-1-1 from your office phone, the correct address and office number is displayed to the 9-1-1
               Telecommunicator. It is also used to place you in a specific group for our emergency alert
               system so that you can be notified by department or location of any event that may affect
               your workplace.

            2.  The Contact Information section tells us how we may contact you for emergency alerts as
               well as non-emergency information. You may be notified by text message or voice recording
               for emergency alerts, and may be contacted by mail with information regarding your
               retirement or benefits.

            3.  The Emergency Contact Information section tells us who should be notified should a personal

               emergency occur.

        Is this secure?
        Yes. The electronic form is encrypted  so that your personal  information provided cannot be
        intercepted by unauthorized persons. This data is considered confidential in nature and will not be
        shared beyond those with a need to know.

        Do I need to do this?
        Yes. Even if your department has previously submitted emergency contact information recently or
        your information was collected previously by the HR department and has not changed, we still need

        you to complete the new form. This allows us to ensure that all information is up to date and you are
        reachable in an emergency. We ask that you take a few moments NOW to complete this short form.
        If we have not received it, we will continue to remind you and your supervisor until it is completed.
