REFERENCETOACOMMITTEEONDISCIPLINE/APPEAL
Section 1. The Office of Student Services through the Discipline Unit shall create anAd Hoc Committee which
shall compose of the Director of Student Services, Director of Instruction/ ARA, two faculty members
and the Chief Justice of theSSC.
TheCommittee onDiscipline shall conduct investigation andcollect pieces of evidence that maybeused as bases of
their findings andrecommendations. Any appeal maybeaddressedto the CampusHeador University President. The
CampusHeador UniversityPresidentmaycreateaCommitteeonAppealswhich shallbetaskedtoreviewthefindings
oftheCommitteeonDiscipline andmakefinal recommendations onthecase.
Section 2. No disciplinary proceedings shall be instituted except for conduct prohibited by law or by the rules and
regulationspromulgated byduly constitutedauthority of theUniversity.
1. Adisciplinary proceeding shall beinstituted upon:
1. Filing of writtencomplaint under oathby thestudent –
complainant to theOfficeof Student Services;
2.Submissionof official report about theviolation of anexistingrules and
regulations.
1. Thereshallbeanofficial entrybookforthepurposeof specifying thepersoncharged,thecomplainantandhis
witnesses andthesubstanceofthe charge.
