Identify the causes:
a) Brainstorm about possible causes.
b) Ask members of the team to spend time between meetings using simple check sheets to gather data, track possible causes and examine the process steps closely.
Construct the actual diagram:
a) Place the problem statement in the box on the right.
b) Draw the traditional major cause categories or other causes that may be useful in your problem analysis.
c) Place the brainstormed ideas in the appropriate major categories in the cause section.
d) For each cause, ask why this happened and list responses as they branch off the major causes.
Interpretation is necessary in order to find the most basic cause of the problem.
a) Look for causes that appear repeatedly.
b) Reach for team consensus.
c) Gatherdatatodeterminetherelevantfrequenciesofthedifferentcauses.

Tips for Effective Cause and Effect Diagrams
Try not to go too far beyond the area that the group can influence in order to minimize frustration.
If ideas are slow in coming, use the major cause categories as springboards.
