MENTOR CO-TRAVEL FORM




       GUIDE TO COMPLETING THE FORM

           Read these instructions for completing the Co-travel form. It is designed to allow you to
           capture observations and provide feedback to the Consultant, Sales Leadership, and Sales
           Education. It is where you will provide information on accounts visited, action items for those
           customers, competitive discoveries, and mentor recommendations.


           You are required to complete the Co-travel form for each day you spend with a Sales
           Consultant. Upon completion, obtain both the consultants and your signature. Then scan and
           email to their Sales Manager and the Sales Education Manager.
