Te Omanga Hospice has continued to grow the capability of our provider partners in the Hutt Valley and Wairarapa over the last year, integrating palliative care across the breadth of health settings: Aged Residential Care facilities, General Practice, and Hutt Valley and Wairarapa Hospitals.

THE TE OMANGA HOSPICE PALLIATIVE CARE IN AGED RESIDENTIAL CARE (ARC)TEAM
SUPPORT FOR PATIENTS IN AGED RESIDENTIAL
Our ARC team of three Clinical Nurse Specialists worked within the 15 aged residential care facilities in the Hutt Valley. They provided support and advocacy to residents and their families
with the transition to aged care, helping with the coordination of their care across many providers. They also provided specialist advice and mentoring to the doctors, nurses, health care assistants, therapists and other care team members in the palliative care work they are already doing.
CARE FACILITIES
   THE TE OMANGA HOSPICE HOSPITAL PALLIATIVE CARE SERVICE
We have provided a Specialist Palliative Care consultation service at Hutt Hospital since
2007. The team consists of a Nurse Practitioner, two Clinical Nurse Specialists and a Palliative Medicine Specialist who assess the palliative care needs of patients in the hospital and advise the hospital clinical teams on appropriate care.

They have continued to strengthen the capability of hospital doctors and nursing teams through the provision of specialist palliative care advice and education on patient care.
