INTEGRATION WITH GPS
Our Palliative Care Facilitators helped to facilitate and coordinate services with the GPs in the Hutt Valley, creating systems that enabled the GPs to support the palliative care needs
of their patients and their whānau. They have continued to strengthen the capability of GPs, Practice Nurses and Palliative Care Link Nurses in the Hutt Valley to continue to provide quality palliative care to their patients.

OUR WORK IN THE WAIRARAPA
We continued to provide our Specialist Palliative Care Medical consultancy service to doctors in the Wairarapa. Working with the Wairarapa Palliative Care Service, our doctors visited weekly and assessed patients referred for consultation by their GP, providing specialist advice on their palliative care and follow-up on-call availability for phone advice.
They also provided specialist education to GPs and other health service providers.
