B. For Old Students

a. The applicants must submit the following:

a. Accomplished application form for scholarship;

b. Photocopy of certification of grades in the preceding semester

from the Office of the University Registrar;

c. Photocopy of the certification of good moral character issued by

the college guidance counselor/college registrar;

d. Photocopy of checklist of course;

e. Photocopy of pre-registration form;

f. 1 pc. 2x2 colored picture (any background color);

g. 1 pcs. short ordinary folder;

h. Specific Requirements;

b. Photocopy of the combined annual family income not exceeding P137,000.00 or

affidavit of non-filling of tax from the Bureau of Internal Revenue (BIR);

(For Job Experience Program applicants only)

c. Photocopy of birth certificate;

(For CvSU Scholars only)

d. Photocopy of Certification of Employment/Service Record of the parent/spouse

from the University Personnel;

(For CvSu Scholarship Privilege applicants only)

e. Photocopy of certification as Barangay Officials/SK Officials from the Department

of Interior and Local Government;

(For RA 7160 (dependents of barangay officials and SK officials

applicants only)

B. For Graduate Students
